Hi! So glad you’re here! Before you proceed with filling out this form, please carefully read the following instructions. Once you submit a booking form, look out for an email from me to continue scheduling!

My minimum is $200 per session and per person. I price by design from there. I require a non-refundable $50 deposit to confirm your appointment. This goes towards the total cost of the session.

Please continue to fill out the form once you have carefully read this page. After filling out the booking form, your appointment will only be confirmed once I have gone through your request and your deposit is made. In your request, please include a detailed descripton of what you are wanting, size in inches, and placement. Also please attach photos for inspiration and reference if available.

* GROUP APPOINTMENTS PLEASE READ

It you and one other, or a small group would like to come in together and are all wanting the same tattoo, please only have one person fill out the booking form. In that form please let me know how many people there will be, everyone's desired tattoo placement, and their info as well (Name, phone number, and email). If you are all wanting different designs, please individually fill out the booking form and let me know the other group members names.

Your appointment will be cancelled if you do not arrive within 15 minutes of your scheduled time. I will not have enough time for your session. You may reschedule your appointment once within at least 48 hours of your appointment. The same deposit will still apply towards your total.

I offer one free touch-up within a year. Anything past a year will require a $50 touch-up fee. Returning clients wanting a touch up, please email me directly to get on my schedule.

*Touch-ups from a different artists work, please email me directly with photos of your tattoo.

Wear the appropriate, comfortable clothing for the location of your tattoo. You may arrive 15 minutes before your scheduled time. You must have a valid form of ID.
